Mhsaa Physical Form 2024-2025

All students wishing to participate in middle school or high school athletics during the 2024/2025 school must have a physical on file dated on or after april 15, 2024 per michigan high school athletic association. Once completed, the signed physical form should be uploaded to the athlete's planeths profile. The michigan high school athletic association, inc., is a private, voluntary association for public, private and parochial secondary schools which choose to join and participate in the organization.
